I'd also like to see it. To upload it you could use a free service such as Box.net. If not I could mock up something in PowerPoint (with Office 2007 ) with what you have in the post.
I created a very rough copy in PowerPoint 2007. It was a bit hard making corrections while keeping true to the original message. I put it up on Box.net if any of you want to download it and make changes to (which I recommend) as it's only the very most basic information with few corrections and probably a few mistakes I left in there myself.
